    Hi Shimam,
    to be hones we use this method quite frequently in patients in whom we do not manage to perform intraluminal recanalization or to get back to the true lumen from above. You could also puncture the popliteal artery but this involves turning the patient and is more cumbersome. When I first performed this type of and approach (after having learned this from Andrej Schmidt, Leipzig), I was astonished how fast this goes.
  • Which vessels are most likely to bleed?

    Supposing that the CT was not helpful in locating the bleeding artery, are there certain pelvic branch vessels that are more likely to bleed than others after trauma?  In other words, are there certain pelvic branch vessels we should look at particularly carefully when doing the angiogram?

    For instance, with an upper GI bleed, we are already suspecting the GDA to be the culprit and will take a good hard look at it during the angiogram.

    We liked this case because it presents a clear-cut example of the value interventional radiology can have in trauma situations. We have been working on streamlining our trauma protocols to ensure that these patients are quickly brought to IR for angiogram and possible intervention. How have other instituitions ensured that the team is ready and available for trauma during daytime and evening hours so these critical patients dont wait?
